Perilymph Fistulae: An Uncommon Cause of Dizziness

چکیده
Dizziness is common complaints that people seek medical attention for. It is listed as one of the top ten symptoms evaluated by primary care providers. To determine the cause of dizziness requires a detailed and careful history and physical examination, with a particular focus on the otolaryngological aspects. A differential diagnosis can be generated from the initial evaluation that can be further corroborated with laboratory tests and imaging studies such as audiograms, video nystagmography, CT scans and MRIs. At this stage, one can often determine whether a patient presents with a vestibular or non-vestibular cause and then refer to the appropriate specialist. To an otolaryngologist, a vestibulopathy is usually apparent in the initial encounter and can be categorized as benign positional vertigo, vestibular neuronitis, viral labyrinthitis, or Meniere’s disease. However, when the patient does not conform to these categories, one must consider other causes, such as atypical or vestibular migraine, superior canal dehiscence syndrome, débarquement syndrome, or perilymph fistula (PLF).

Noise-induced perilymph fistula.
Perilymph fistulae are difficult to diagnose because they present with a wide variety of signs and symptoms, they are associated with many etiologies, and they often mimic other conditions. In this article, we describe a case of perilymph fistula that featured one of its more rare causes: acoustic trauma--specifically, damage from a loud blast from the siren of a fire engine. We also review the...

Perilymphatic fistula has been believed to cause acute, profound and reversible sensory hearing loss. Over the past five years, exploratory tympanotomy were performed in 27 patients with perilymphatic fistula. In three patients, air bubbles came out of the round window niche in the course of perilymphatic leakage. The perilymphatic fistulae observed in these three patients were attributable to ...
متن کاملA Case of Intractable Suspected Perilymph Fistula with Severe Depression
A 68-year-old woman presented dizziness whenever she put her finger into the right ear and also complained of water-streaming tinnitus, which indicated she would have been suffering from perilymph fistula. An exploratory tympanotomy was conducted. Leakage of perilymph from the round window was suspected, although the cochlin-tomoprotein (CTP) results were negative. After the procedure, the pati...
متن کاملDiagnosing the cause of vertigo: a practical approach.
Dizziness is among the commonest of chief complaints. It often presents a significant challenge to the attending physician, because the symptoms and signs are often vague and non-specific. However, a robust systematic approach can usually arrive at the diagnosis. Maintaining balance requires sensory inputs from the vestibular, visual, and somatosensory systems and the cerebellum fine-tunes inac...
متن کاملA Third Labyrinthine Window: An Overview of Perilymph and Labyrinthine Fistulae and Superior Semicircular Canal Dehiscence
The membranous labyrinth is contained within the bony labyrinth and surrounded by perilymph. The only two 'potentially yielding' parts of the otherwise solid bony labyrinth are the oval and round windows, which by their relative movements, pressure differentials and resilience are responsible for all the functions attributed to the inner ear. In pathologies, such as trauma, infection or occasio...
